Fagita Lekoma Woreda

In the case of Fagita Lekoma Woreda in the Awi Zone of the Amhara Region:
The woreda is created under three different names on the eCHIS dashboard Fagita, Fagita/Fagita, and Fagita Lekoma. As a result, some health posts are registered and appear under all three categories, leading to inconsistent results being displayed across the different administrative hierarchies.

Due to this problem health posts are not properly linked with their respective or nor appeared in their PHCU as well as they couldn’t link clients or the referral application not working functional

Proposed solution : We communicated with the Woreda Health Office and the Zonal Health Department regarding the issue, and they agreed to merge the three names into a single, correct name- consistent with DHIS2 - as “Fagita Lekoma Woreda”

The same procedure should be followed for health pots and health center link (MFR) and be based on the MFR on DHIS2
