Error during accepting action card from health center

Dear Support Team,
We are experiencing issues when trying to accept action cards from the following health centers:-

1. Oromia Region, Buno Beddele Zone, Chora Woreda, Kiltu Shibo Health Center…Dabes Soro HP.
2. Oromia Region, Buno Beddele Zone, Chora Woreda, Abdella Health Center…Ale Sardo HP .
